Women from all types of backgrounds may choose to have an abortion (also known as a termination) in the UK. It is legal up to 24 weeks, or after 24 weeks in limited circumstances (for example, if there is a risk to the mother’s life), in line with the Abortion Act 1967. Whatever your circumstances, it is crucial to understand your options when considering an abortion, including the types of abortion that you can undergo.
The type of abortion available to you is largely determined by how long you have been pregnant. There are two types of abortion:
- Medical – this involves using medication to terminate the pregnancy. Depending on gestation, you may be eligible for pills by post.
- Surgical – this will require you to undergo a small operation to terminate the pregnancy
Mr Alex Steshenko, Consultant Gynaecologist at The Gynae Centre, says, “There are many reasons why someone may wish to have an abortion, but regardless of their circumstance, it is important they understand what’s involved when it comes to the different abortion procedures, and which of these procedures are available to them. There is plenty of information available on reputable websites, but if there’s still some uncertainty, a consultant gynaecologist will be able to advise.”
The type of abortion you have may be influenced by the timeline of your pregnancy, so it’s important to be timely in seeking advice from a consultant gynaecologist if you are unsure about your next steps. However, it’s also essential to know that you should never feel pressured – by a medical professional or anyone else – to undergo or not undergo any procedures when it comes to your body. At The Gynae Centre, our consultants are unbiased, discreet, and sympathetic when providing medical advice.
Medical abortion (The abortion pill)
How it works
A medical abortion uses two different medications to end a pregnancy. The first tablet, mifepristone, is taken to stop the pregnancy from continuing. This is followed 24-48 hours later by misoprostol, which causes the womb to contract and expel the pregnancy tissue. The process is similar to a miscarriage and takes place without surgery.
When it’s used
Medical abortion is typically offered in early pregnancy, usually up to 10 weeks. It may be recommended for those who prefer a non-surgical option or wish to manage the process in a more private setting. You will attend the clinic for your consultation and to take the first pill. In some instances, for example if there is uncertainty around conception dates, you may also need an ultrasound scan as a safety precaution.
The experience
After taking the second medication, most people experience cramping and bleeding, which can range from mild to heavy. This usually starts within a few hours but can take longer for some. Pain relief is provided, and bleeding often tapers off over several days, although lighter bleeding or spotting can continue for a short time. Follow-up guidance is given to confirm the abortion has been successful and to support recovery.
Medical abortion pills by post
How it works
Medical abortion pills by post allows a remote medical abortion to take place at home. After a telephone consultation with an experienced gynaecologist, the medication is sent discreetly by courier. The process uses the same two-step medication method as an in-person medical abortion, but both pills can be taken from home.
When it’s used
This option is suitable for pregnancies up to 10 weeks, following an assessment by a consultant. It may not be appropriate in all circumstances, for example if there is uncertainty around conception dates or menstrual irregularity, in which case an ultrasound scan may be recommended.
The experience
You will have a telephone consultation with a gynaecologist who will discuss your medical history, confirm gestation, and ensure this is the right option for you. Once approved, the medication is delivered within 2-3 working days. Clear, step-by-step instructions are provided on how and when to take each tablet, what to expect, and how to manage pain and side effects. Ongoing support is available by phone and email, with emergency contact details provided for reassurance throughout the process.
Surgical abortion
How it works
A surgical abortion is a procedure carried out by a specialist to remove the pregnancy from the womb. It is completed using a level of suction determined by the stage of pregnancy. The procedure itself is short and is performed by experienced clinicians.
For pregnancies under 15 weeks, Manual Vacuum Aspiration (MVA) is used. This is one of the least invasive surgical options available and is carried out using a small, handheld vacuum device to create gentle suction.
For pregnancies of 15 weeks and over, Electric Vacuum Aspiration (EVA) is used. This method uses stronger suction to ensure the procedure is completed efficiently and to help reduce the risk of excessive bleeding.
When it’s used
Surgical abortion may be offered at later gestations or when a medical abortion is not suitable or preferred. Some people choose this option because it is quicker and usually completed on the same day.
The experience
The procedure is carried out in a controlled medical setting, with appropriate pain relief or sedation discussed beforehand. Most people are able to go home the same day. Cramping and light bleeding are common afterwards, but usually settle within a few days. Aftercare advice and follow-up support are provided to ensure physical recovery and wellbeing.
Types of abortion: Which one is right for me?
There is no single ‘right’ option when it comes to the types of abortions available to you. The most suitable choice depends on how far along the pregnancy is, your medical history, and what feels right for you physically and emotionally. Some people prefer the privacy of pills by post, while others feel more comfortable with a procedure carried out in a clinical setting. Both are safe, legal options in the UK when provided by experienced specialists.
